This worksheet is intended for use by employees and supervisors in organizations that seek to foster or maintain wellness in their workplace. Staff can complete the worksheet in order to reflect on their role in promoting wellness in the workplace using the Eight Dimensions of Wellness. The Eight Dimensions of Wellness are emotional wellness, environmental wellness, financial wellness, intellectual wellness, occupational wellness, physical wellness, social wellness, and spiritual wellness. A definition for each dimension is given to those not familiar with the model, as well as a visual guide to demonstrate how the dimensions overlap.

The worksheet prompts employees to reflect on the dimensions of wellness that are most important to them, why they are most important, how those dimensions could be better supported in the workplace, and ways in which employees could bring those ideas to their organization’s leadership. This two-page resource is free to download. Examples are provided on the second page to help employees conceptualize ways to foster wellness in their place of work.
